Know All Men By These Presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland made this day of April 2006, by me the undersigned Jay Ross of 3 Dean Swift Mews, Markethill, Armagh, County Armagh, married now or lately called Jennifer Ann Ross a commonwealth citizen.

WITNESSETH AND IT IS HEREBY DECLARED AS FOLLOWS:-

1.	I absolutely and entirely renounce relinquish and abandon the use of my said former Christian names of Jennifer Ann and assume adopt and determine to take and use the name of Jay in substitution for my former Christian names of Jennifer Ann.
2.	I shall at all times here after in all records deeds documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Jay as my Christian name in substitution for my former Christian names of Jennifer Ann so relinquished as aforesaid to the intent that I may hereafter be called known or distinguished not by the former Christian names of Jennifer Ann but by the Christian name of Jay only.
3.	I authorise and require all persons at all times to designate describe and address me by the adopted Christian name of Jay.
