Take Notice that by Deed Poll executed on the 18th day of May 2006 and enrolled in the Supreme Court of Judicature in Northern Ireland on the 13th day of June 2006 that Leanne Ross, single on behalf of Ross William Lee Collins, c/o Stewarts Solicitors, 3 Regent Street, Newtownards, Co Down, BT23 4AB a minor, absolutely and entirely renounced, relinquished and abandoned the use of his former surname of Gourley and assumed, adopted and determined to take and use the surname of Ross William Lee Collins in substitution for his former surname of Gourley.
