I hereby give notice that by a Change of Name Deed dated 14 June 2001 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 2LL, on 25 July 2001, I Ama Baiden, of 17 Grosslea Court, Bishopsford Road, Morden, Surrey SM4 6BE, the mother of Eric Kwesi Obiri Asante, an unmarried child and a British citizen renounced relinquished and abandoned the name of Eric Kwesi Obiri Asante Addo and assumed the name of ERIC KWESI OBIRI ASANTE.
