Notice is now given by a Deed Poll dated 16 September 2003 and enrolled in the Central Office of the Supreme Court on 7 October 2003, I, PERRY MARK FIELD, of 6 Marlborough, Maida Vale, London W9 1RW, a British citizen, under the British Nationality Act 1981, section 11(1), formally and absolutely renounced and abandoned the surname of Shoefield and have assumed and adopted and intend on all occasions to use and subscribe the name of Field instead of the surname Shoefield so as to be at all times called, known and described by the name of Perry Mark Field exclusively.
