Notice is hereby given that by a Deed Poll dated 21 July 2005 and enrolled in the Central Office of the Supreme Court on 26 July 2005, I, EDDIE ZAKAY, of 55 Woodstock Road, London NW11 8QD, married and a British citizen under the British Nationality Act 1981, section 6(1) formally and absolutely renounced and abandoned the first name of Ezra and have assumed and adopted and intend on all occasions to use and subscribe the first name of Eddie instead of the first name of Ezra so as to be at all times called known and described by the name of Eddie Zakay exclusively.
