I hereby give notice that by a Deed Poll dated 25 September 2006 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL, I, Kim Isabella Ford, of 25 Rosetta Road, Southsea, Hampshire PO4 8JY, the mother of HARVEY STERLING FORD, a child and a British citizen renounced, relinquished and abandoned the name of Harvey Sterling Johnson and assumed the name of Harvey Sterling Ford.
