KNOWN ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Deborah Elizabeth Topping of 15 Kyle Avenue, Lurgan, in the County of Armagh, single, Administrative Assistant now or lately called Deborah Elizabeth Heron a commonwealth citizen.

Witnesseth and it is declared as follows:

1. I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Heron and assume, adopt and determine to take and use the surname of Topping in substitution for my former surname of Heron.

2. I shall at all time herein in all records, deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Topping as my surname in substitution for my former surname of Heron so relinquished as aforesaid to the intent that I may hereinafter be called, known or distinguished not by the former surname of Heron but by Topping only.

3. I authorise and require all persons at all times to designate, describe and address me to the adopted surname of Topping.
