Know all men by these presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Elizabeth Dodwell Playle of The Manor House, Dundrum, County Down, Married Woman now or lately called Gladys Elizabeth Playle a Commonwealth Citizen.

Witnesseth and it is hereby declared as follows:-

I absolutely and entirely renounce relinquish and abandon the use of my said Christian or first name of Gladys Elizabeth and assume adopt and determine to take and use the Christian or first name of Elizabeth Dodwell in substitution for my former Christian or first name of Gladys Elizabeth. I shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said names Elizabeth Dodwell as my Christian or first names in substitution for my former Christian or first names of Gladys Elizabeth so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former Christian or first names of Gladys Elizabeth but by the Christian or first names of Elizabeth Dodwell only.

I authorise and require all persons at all times to designate describe and address me by the adopted Christian and first names of Elizabeth Dodwell.

In Witness whereof I have hereunto subscribed my adopted and substituted Christian and first names of Elizabeth Dodwell and my surname of Playle and also my said former Christian or first names of Gladys Elizabeth and have affixed my seal this 7th day of December, 2002.
