Known all men by these presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Paula Lavery of 122 Park Road, Portadown in the County of Armagh, divorced, waitress now or lately called Paula McKinstry, a commonwealth citizen.

Witnesseth and it is declared as follows:

1.	I absolutely and entirely renounce relinquish and abandon the use of my said former surname of McKinstry and assume, adopt and determine to take and use the surname of Lavery in substitution for my former surname of McKinstry.
2.	I shall at all time herein in all records, deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Lavery as my surname in substitution for my former surname of McKinstry so relinquished as aforesaid to the intent that I may hereinafter be called, known or distinguished not by the former surname of McKinstry but by Lavery only.
3.	I authorise and require all persons at all times to designate, describe and address me to the adopted surname of Lavery.

In Witness Whereof I have hereunto subscribed my Christian or first name of Paula and my adopted and substituted surname of Lavery and also my said former surname of McKinstry and have affixed my seal this 8th day of July, 2003.
