KNOW ALL MEN BY THESE PRESENT (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Beverley Nicola Milliken of 43 Lower Windsor Avenue, Lisburn Road, Belfast BT9 7DW, single, unemployed person now or lately called Beverley Nicola Byrne a Commonwealth Citizen.
WITNESSETH AND IT IS HEREBY DECLARED as follows:–
I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Byrne and assume adopt and determine to take and use the surname of Milliken in substitution for my former Surname of Byrne.
I shall at all times hereafter in all records, deeds, and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Milliken as my surname in substitution for my former surname of Byrne so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Byrne but by the surname of Milliken only.
I authorise and require all persons at all times to designate, describe and address me by the adopted surname of Milliken.
IN WITNESS whereof I have hereunto subscribed by Christian or first names of Beverley Nicola and my adopted and substituted surname of Milliken and also my said former surname of Byrne and have affixed my seal this 17th day of May 2005.
