Notice is hereby given that I, Jayne Claire Hughes, the mother of NICOLA JADE HUGHES, formerly called and known by the name of Nicola Jade Gunn, a citizen of the United Kingdom by birth, give public notice that, from 13th October 1999, I formerly and absolutely renounced and abandoned the use of Nicola’s surname of Gunn and assumed and adopted in place thereafter the surname of Hughes and from this day forward I instruct all persons to address her by her name of Nicola Jade Hughes and further that such change of name is evident by Deed Poll dated 13th October 1999 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature.
