Notice is now given that by a Deed Poll dated 23rd February 2001 and enrolled in the Central Office of the Supreme Court on 25th April 2001 by Sarah Helen Chinn, having parental responsibility for BETHANY ELIZABETH CHINN, of 19 Higher Peverell Road, Penzance, Cornwall, a British citizen, under the British Nationality Act 1981, section 1(1), abandoned on behalf of Bethany Elizabeth Chinn the surname of Fortune and assumed in lieu the surname of Chinn.
