Notice is hereby given that by a Deed Poll dated 13 June 2001 and enrolled in the Supreme Court of Judicature on 27 July 2001, Sadie Jane Gooch, as person having parent responsibility, on behalf of DANIEL JAY GOOCH, a child, single and a British citizen, under section 1(i) of the British Nationality Act 1981, abandoned the surname of Bennet and assumed in lieu thereof the surname of Gooch and added a middle name Jay to read Daniel Jay Gooch.
