Notice is hereby given that by Deed Poll dated 15 August 2002 and enrolled in the Supreme Court of Judicature on 8 October 2002, as the Parent Mandy Louise Smith having parental responsibility on behalf of MAXIMUS HARRISON SMITH, a child, single and a British citizen under section 11(1) of the British Nationality Act 1981, abandoned the surname of Mosby and assumed in lieu thereof the surname of Smith.
