I hereby give notice that by a Deed Poll dated 21 May 2003 and enrolled at the Central Office of the Supreme Court of Judicature, Royal Courts of Justice, Strand, London WC2A 2LL, on 6 June 2003, I, Diane Isobel Donaldson, of 30 Broadway Avenue, St Margarets, Twickenham, Middlesex TW1 1RH, the mother of Gemma Jade Donaldson, an unmarried child and a British citizen, by virtue of section 1 of the British Nationality Act 1981, renounced and relinquished and abandoned the name of Gemma Jade Collier and assumed the name of GEMMA JADE DONALDSON.
