Notice is hereby given that I, JONATHAN ROBERT TEBB-ROGER, of South View, Church Houses, Farndale, Kirkbymoorside, York YO62 7LF, formerly called and known by the name of Jonathan Robert Roger, a British citizen of the United Kingdom by birth, give public notice that, on 23 May 2003, I formally and absolutely renounced and abandoned the use of my surname of Roger and assumed and adopted in place thereof the surname of Tebb-Roger and further that such change of name is evidenced by a Deed Poll dated 23 May 2003 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court on 5 June 2003.
