I hereby give notice that by a Deed Poll dated 18 June 2003 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL on 27 June 2003, I, Joanne Michelle Smith, of 43 Mayfield Avenue, Mayfield, Ashbourne, Derbyshire DE6 2JB, the mother of WILLIAM PATRICK SMITH, an unmarried child and a British citizen, renounced, relinquished and abandoned the name of William Patrick Mace and assumed the name of William Patrick Smith.
