Notice is hereby given that by a Deed Poll dated 13 August 2003 and enrolled in the Supreme Court of Judicature on 26 September 2003, I, Lisa Reynolds, of 18 Chesford Grove, Stratford upon Avon CV37 9LS, as person having parental responsibility, on behalf of MAYA ESME ANNE REYNOLDS, a child, single and a British citizen, abandoned the surname of Reynolds-Wallis and assumed in lieu thereof the surname of Reynolds. 13 August 2003.
