Notice is now given that by a Deed Poll dated 24 May 2006 and enrolled in the Central Office of the Supreme Court on 7 June 2006, I, MUKTABEN THAKERSHI HATHI, of 40 Briarwood Drive, Northwood, Middlesex HA6 1PL, widowed, a British Overseas citizen under the British Nationality Act 1981 section 6, formally and absolutely renounced and abandoned the surname of Kalidas and the forename of Muktabai and have assumed and adopted and intend on all occasions to use and subscribe the surname of Hathi instead of the surname of Kalidas and the forename of Muktaben instead of the forename of Muktabai so as to be at all times called known and described by the name of Muktaben Thakershi Hathi exclusively.
