1. Olasumbo Ganiat Lawal of Flat 24 Cobham House, Boundary Street, Erith, Kent DA8 2ES as a person having parental responsibility for Zainab Titilope Oluwaseyi Umar renounced and abandoned, on behalf of Zainab Titilope Oluwaseyi Umar, the name of Zainab Titilope Oluwaseyi Lawal, and in place thereof assumed, on behalf of Zainab Titilope Oluwaseyi Umar, the name of Zainab Titilope Oluwaseyi Umar.

2.  Zainab Titilope Oluwaseyi Umar is single.

3.  Zainab Titilope Oluwaseyi Umar is a British citizen under the British Nationality Act 1981 section 1(1).
