Person/persons having parental responsibility on behalf of the said JOSEPH JAMES mcphillips who is a child, single and a Commonwealth/British Citizen under Section 11(1) of the British Nationality Act 1981 abandoned the name of Joseph James Kotb and assumed the name of Joseph James McPhillips.
