Notice is hereby given that by a Deed Poll dated 23 June 2004 and enrolled in the
Supreme Court of England and Wales on 7 July 2004, I, Keisha Annmarie Palmer, of 323
Lodge Lane, New Addington CR0 0DQ, the person having parental responsibility for TABITHA
DE-CHANEL PALMER who is 5 years of age, single and a British citizen by virtue of
section 1(1) of the British Nationality Act 1981, abandoned the name of Waneka De-Chanel
Palmer and assumed in lieu thereof the name of Tabitha De-Chanel Palmer.
