Notice is hereby given that a Deed Poll dated 3 August 2004 and enrolled in the Supreme Court of Judicature on 12 October 2004, I, Dawn Downes, of 5 Nodder Road, Woodthorpe, Sheffield S13 8DB, as the person having parental responsibility on behalf of the said JADE KEELY SYKES, who is a child, single and a Commonwealth citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Jade Keely Walker and assumed the name of Jade Keely Sykes.
