Notice is hereby given that by a Deed Poll dated 26 January 2006 and enrolled in the Supreme Court of Judicature on 3 March 2006, Kate Emily Anne Crouch as a person having parental responsibility for LEWIS ALLAN CROUCH, a child being single and a British citizen under section 11 of the British Nationality Act 1981, abandoned the surname of Surrey and assumed in lieu thereof the surname of Crouch.
