I hereby give notice that by a Deed Poll dated 18 February 2008 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL, I, KATIE ANN VAVERE-DAVIES of 7 Old Castle, Guilsfield, Welshpool, Powys SY21 9PA a British citizen, renounced, relinquished and abandoned the name of Katie Ann Davies and assumed the name of Katie Ann Vavere-Davies.
