Notice is hereby given that a Deed Poll dated 23 January 2014 and enrolled in the Senior Courts of England and Wales on 14 April 2014, as persons formerly having parental responsibility on behalf of Chantelle Louise Witt, who is a child, single a British Citizen under section 1(1) of the British Nationality Act 1981 abandoned the name of Chantelle Louise Donoghue and assumed the name of Chantelle Louise Witt.
