Change of Name by Deed Poll

Notice is hereby given that by a Deed Poll dated the 2nd day of July 2002, and enrolled in the Supreme Court of Judicature on the 16th day of September 2002, as Tracey Taggart, mother having parental responsibility on behalf of Jessica Heather Reilly, a child, single and a Commonwealth citizen, abandoned the surname of Reilly and assumed in lieu thereof the surname of Taggart.
