Know all men by these presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Jacqueline Eileen Smith of 36 Broomhill, Portadown, Craigavon in the County of Armagh, Married Woman, being a Care Assistant and the legal guardian of Jonathan Alexander Smith whose date of birth is 6th May 1986 now or lately called Jonathan Alexander Steele a Commonwealth Citizen on behalf of the said Jonathan Alexander Smith.

Witness and it is hereby declared as follows:-

On behalf of Jonathan Alexander Smith I absolutely and entirely renounce, relinquish and abandon the use of his former surname of Steele and on his behalf assume, adopt and determine to take and use the surname of Smith in substitution for his former surname of Steele. That the said Jonathan Alexander Smith shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Smith as his surname in substitution for his former surname of Steele so relinquished as aforesaid to the intent that he may hereafter be called, known or distinguished not by the former surname of Steele but by the surname of Smith only.

I hereby on behalf of the said Jonathan Alexander Smith authorise and require all persons at all times to designate, describe and address him by the adopted surname Smith.
