This Deed is dated the 28th day of April 2004.

KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Eilish Dysart of 19 Killowen Street, Coleraine in the County of Londonderry, Cleaner, who has obtained a Divorce from Gregory Russell Stewart, a Commonwealth Citizen.

WITNESS AND IT IS HEREBY DECLARED as follows:

1. I absolutely and entirely renounce, relinquish and abandon the use of my said former surname of Stewart and assume adopt and determine to take and use the surname of Dysart in substitution for my former surname of Stewart.

2. I shall at all times hereafter in all records, Deeds, Documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribed the said name of Dysart as my surname in substitution for my former surname of Stewart so relinquished as aforesaid to the intent that I may hereafter be called known or distinguished not by the former surname of Stewart but by the surname of Dysart only.

3. IN WITNESS whereof I have hereunto subscribed my Christian or first names of Eilish and my adopted and substituted surname of Dysart and also my said former surname of Stewart and affixed my seal the day and year first above written.
