Down BT23 4AB, divorced, absolutely and entirely renounced, relinquished and abandoned the use of her former christian name of Cheryl and former surname of Sedgwick and assumed, adopted and determined to take and use the christian name of Cherry and the surname of Benstead in substitution for her former christian name of Cheryl and surname of Sedgwick.
