KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned DONNA LOUISE NEILL a Student of 36 Ballyskeagh Road, Lambeg, Lisburn in the County of Antrim, a Spinster now or lately called DONNA LOUISE BOYCE a Commonweath Citizen.

WITNESSETH AND IT IS HEREBY DECLARED as follows:

I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Boyce and assume adopt and determine to take and use the surname of Neill in substitution for my former surname of Boyce.

I shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Neill as my surname in substitution for my former surname of Boyce so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Boyce but by the surname of Neill only.

I authorise and require all persons at all times to designate describe and address me by the adopted surname of Neill.

IN WITNESS whereof I have hereunto subscribed my Christian or first names of Donna Louise and my adopted and substituted surname of Neill and also my said former surname of Boyce and have affixed my seal this 18th day of May2005.
