KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Elizabeth Margaret Janice Taylor-Clarke of 2 Holland Gardens, Belfast, Married Woman, Assistant Case Officer the legal guardian of Elizabeth Ann Lita Taylor-Clarke born on 3 November 2000 now or lately called Elizabeth Ann Lita Clarke a Commonwealth Citizen on behalf of the said Elizabeth Ann Lita Taylor-Clarke.

WITNESS AND IT IS HEREBY DECLARED as follows:-

On behalf of Elizabeth Ann Lita Taylor-Clarke I absolutely and entirely renounce relinquish and abandon the use of her former surname of Clarke and on her behalf assume adopt and determine to take and use the surname of Taylor-Clarke in substitution for her former surname of Clarke. That the said Elizabeth Ann Lita Taylor-Clarke shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Taylor-Clarke as her surname in substitution for her former surname of Clarke so relinquished as aforesaid to the intent that she may hereafter be called, known or distinguished not by the former surname of Clarke but by the surname of Taylor-Clarke only.

I hereby on behalf of the said Elizabeth Ann Lita Taylor-Clarke authorise and require all persons at all times to designate describe and address her, by the adopted surname of Taylor-Clarke.
