Notice is hereby given that by a Deed Poll dated the 1st October 1998 and enrolled in the Supreme Court of Judicature on 1st December 1998, DEENA LORRAE WARK, who resides at Wernllwyn, Temple Bar, Lampeter in the county of Ceredigion, as person having parental responsibility, on behalf of LEE JOHN LLOYD, a child, single and a British citizen abandoned the surname of Wark and assumed in lieu thereof the surname of Lloyd.
