Notice is hereby given that by a Deed Poll dated 7th June 1999, and enrolled in the Supreme Court of Judicature on 25th June 1999, as Emma Page, a person having parental responsibility, on behalf of JACK DICK, of 22 Willowmead, Leybourne, West Malling, Kent M19 5RE, a child, single, and a British citizen, abandoned the surname of Page and assumed in lie thereof the surname of Dick.
