Notice is hereby given that I, REZENE ABRAHAM, of 23 Cricklewood Lane, London NW2 1HN, formerly called and known by the name of Cawad Ibrahim Musse, a citizen of Somalia by birth, give public notice that, on 11 June 2001, I formally and absolutely renounced and abandoned the use of my first name and surname of Cawad Ibrahim Musse and assumed and adopted in place thereof the first name and surname of Rezene Abraham and further that such change of name is evidenced by a Statutory Declaration dated 11 June 2001, under my hand and duly executed by me and attested by a Solicitor/Commissioner of Oath on 11 June 2001.
