D J COOK LIMITED RETIREMENT BENEFITS SCHEME
Notice is hereby given to all parties who are or were members of the D J Cook Limited Reitrement Benefits Scheme (“the Scheme”) that it is to be wound up. It is believed that all members of the Scheme and their benefits under it have been identified and the Trustees announce their intention to distribute the remaining assets of the Scheme. If you were a Member of the Scheme and believe you may be entitled to benefits from it and you have not received an announcement dated 9 October 2001, please post notification of your claim specifying your full name, address and particulars of your membership of the Scheme to the Trustee, Burges Salmon Pension Trustees Limited (Ref SJ03), Narrow Quay House, Narrow Quay, Bristol BS1 4AH, so as to reach that address on or before 13 February 2002. Claims received after that date will be excluded from benefits under the Scheme. 12 December 2001.
