Notice is hereby given that I, John Farah Taylor, of 19 Portland Road, Hayes, Middlesex UB4 8LD, formerly called and known by the name of Anwar Ali and then John Farah, a citizen of the United Kingdom by birth, give public notice that, on 30 August 2002, I formally and absolutely renounced and abandoned the use of my name of Anwar Ali as well as John Farah and assumed and adopted in place thereof the name of JOHN FARAH-TAYLOR and further that such change of name is evidenced by a Deed Poll dated 30 August 2002 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 5 September 2002.
