Notice is hereby given that by a Deed Poll dated 10 December 2002 and enrolled in
the Supreme Court of Judicature on 3 February 2003, Alexandra April Naomi Lavery and
Andrew David Lavery, the parents of EDWARD JAMES ISAAC REUBEN LAVERY, of 14 Gainsborough
Gardens, London NW3 1BJ, a minor and a British citizen, under section 1(1) of the
British Nationality Act 1981, abandoned the surname of Lavy and assumed the surname
of Lavery on behalf of their son.
