Notice is hereby given that I, KIDIST WOLDEYES, of Flat 22, Brockworth, Gloucester Road, Kingston, Surrey KT1 3RG, formerly called and known by the name of Kidist Negash a citizen of Ethiopia by birth, give public notice that, on 6 May 2003, I formally and absolutely renounced and abandoned the use of my surname of Negash and assumed and adopted in place thereof the surname of Woldeyes and further that such change of name is evidenced by a Statutory Declaration dated 6 May 2003 under my hand duly executed by me and attested by a Solicitor/Commissioner of Oath on 6 May 2003.
