Notice is now given that by a Deed Poll dated 20 November 2003 and enrolled in the Central Office of the Supreme Court on 20 November 2003, ANNA KRYSTYNA OGSTON, of 11 Pitt Street, London, a Commonwealth citizen, formally and absolutely renounced and abandoned the surname of Derkacz and assumed and adopted and intends on all occasions to use and subscribe the surname of Ogston instead of the surname of Derkacz so as to be at all times called, known and described by the name of Anna Krystyna Ogston exclusively.
