Notice is now given that by a Deed Poll dated 9 August 2003 and enrolled in the Central Office of the Supreme Court on 16 January 2004, I, DOMINIKA MAGDALENA FIRTH, of 43 Russetts, Langdon Hills, Basildon, Essex SS16 6SH, formerly known as Dominika Magdalena Zietala, single and a British citizen, under the British Nationality Act 1981, section 3(1), formally and absolutely renounced and abandoned the surname of Zietala and have assumed and adopted and intends on all occasions to use and subscribe the surname of Firth instead of the surname of Zietala so as to be at all times called, known and described by the name of Dominika Magdalena Firth exclusively.
