Notice is hereby given that by a Deed Poll dated 10 December 2003 and enrolled in the Supreme Court of Judicature on 5 March 2004, Gary Healey, as person having parental responsibility, on behalf of EMMA LOUISE HEALEY, of 22 Johnston Avenue, Prescot L35, a child, single and a British citizen, under section 1(1) of the British Nationality Act 1981, abandoned the surname of O’Neill and assumed in lieu thereof the surname of Healey.
