Notice is hereby given that I, Eileen Theresa Milligan, of 38 Stilecroft Gardens, Wembley, Middlesex HA0 3HD, mother of SOPHIE MONICA MILLIGAN-BROWN, formerly called and known as Sophie Monica Brown, a citizen of the United Kingdom by birth, give public notice that, on 22 April 2004, my daughter Sophie Monica Milligan-Brown absolutely renounced and abandoned the use of the surname Brown and assumed and adopted in place thereof the surname of Milligan-Brown and further that such change of name is evidenced by a Deed Poll dated 22 April 2004 under my hand duly executed by me and attested and enrolled in the enrolment department of the Central Office of the Supreme Court of Judicature on 19 May 2004.
