Notice is hereby given that by a Deed Poll dated 16 June 2004 and enrolled in the Supreme Court of Judicature on 16 June 2004, John Phillip Oldridge and Jacqueline Anne Oldridge, as persons having parental responsibility on behalf of the said SAMUEL JOHN OLDRIDGE, of 231 Sandbanks Road, Lilliput, Dorset BH14 8EY, who is a child, single and a British citizen, under section 1(1) of the British Nationality Act 1981, abandoned the name of Samuel James Oldridge and assumed the name of Samuel John Oldridge.
