I hereby give notice that by a Deed Poll dated 13 July 2004 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL, on 10 March 2005, I, Jacqueline Madden, of 9 King Albert Court, King Albert Street, Landport, Portsmouth, Hampshire, the mother of AMY LOUISE MADDEN and FAY LEANNE MADDEN, British citizens, under section 1(1) of the British Nationality Act 1981, renounced and relinquished and abandoned the names of Amy Louise Ruddock and Fay Leanne Ruddock and assumed the names of Amy Louise Madden and Fay Leanne Madden.
