I hereby give notice that by a Deed Poll dated 25 November 2004 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL, on 13 April 2005, I, Karen Ann Whitfield, of Flat 6, 99-105 Victoria Road North, Southsea, Portsmouth, Hampshire PO5 1PS, the mother of MACHALA JADE WHITFIELD, a British citizen, renounced and relinquished and abandoned the name of Machala Jade Zimmerman born as Machala Jade White and assumed the name of Machala Jade Whitfield.
