Notice is hereby given that by a Deed Poll dated 12 August 2005 and enrolled in the Supreme Court of England and Wales on 12 August 2005, I, RITA E UNDERHILL-SMITH, of Flat 10, 1 Osbert Street, London SW1P 2QU, single and a British citizen by virtue of section 11(1) of the British Nationality Act 1981, abandoned the name of Rita Elizabeth Smith and assumed in lieu thereof the name of Rita E Underhill-Smith.
