Notice is hereby given that a Deed Poll dated 1 July 2008 and enrolled in the Supreme Court of Judicature on 11 July 2008, A Stokes and M Stokes, as persons having parental responsibility on behalf of the said Levi Stokes, who is a child single, and a Commonwealth/British citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Levi Mark Whitaker and assumed the name of Levi Stokes.
