Notice is hereby given that by a Deed Poll dated 6 August 2009 and enrolled in the Supreme Court of Judicature on 6 August 2009, we, Elizabeth Ann Smith and John Montague Smith of Sundance, Thicket Grove, Maidenhead, Berkshire SL6 4LW, as persons having parental responsibility for Spike John Victory Alexander Smith who is a child single and a British citizen under section 1(1) of the British Nationality Act 1981 on behalf of the said Spike John Victory Alexander Smith abandoned the name of John Harry Alexander Smith and assumed the name of Spike John Victory Alexander Smith.
