Notice is hereby given that a Deed Poll dated 14 December 2009 and enrolled in the Supreme Court of Judicature on 11 March 2010 as person having parental responsibility on behalf of the said Sara Jane Smith who is a child single and a Commonwealth/British Citizen under section 1(1) of the British Nationality Act 1981 abandoned the name of Sara Jane Thomas and assumed the name of Sara Jane Smith.
